Olympic Medical Center physicians Dr. Mark Lacambra, Dr. Vara Kraft and Dr. Kim Wheeling are currently accepting new children and adult patients.

Forms

Please download and complete the New Patient Packet and bring it with you to your new patient visit. Please arrive 15 minutes prior to your appointment time to finish the registration process. Reading and completing the forms prior to your appointment will reduce your wait time.

Your New Patient Packet includes:

• welcome letter
• patient registration form
• patient health questionnaire
• financial policy
• patient information release form
• advance directive information sheet

Insurance

Olympic Medical Center accepts most health insurance plans. We strongly encourage you to contact your insurance company to confirm the physician you have chosen is a provider on your plan and that you understand your covered benefits prior to your appointment time.

Lab services not provided by our facility are sent to an outside laboratory and are billed directly from Northwest Clinical Labs or Cellnetix. Northwest Clinical Labs services are billed under outpatient hospital services. Please familiarize yourself with your insurance coverage and benefits on this service before scheduling your appointment.

Hospitalization

Olympic Medical Center patients in need of hospitalization will be admitted to Northwest Hospital & Medical Center unless requested otherwise.

Directions from 1-5 North- or South-bound:

1. Take the 85th Street Exit and head west on 85th Street
2. Take a left at light on 24th Avenue (approximately 2 miles)
3. Proceed through the next traffic light and
4. We are located ½ block down on the right-hand side
5. Parking is on the North side of the building

Directions from Aurora/99 North- or South-bound:

1. Head West on 80th Street (take a right from South-bound and a left from North-bound)
2. Take a left at the light on 24th Avenue
3. We are located ½ block down on the right-hand side
4. Parking is on the North side of the building

By Bus:

• We are located directly on the 18 and 75 bus-lines.
• We are also located on the 48-bus line, but it requires a transfer to the 75-bus line at 85th street.

Our building is wheelchair accessible from the parking lot.
